List of attachments needed:
- Details of posted employees (Click here for form)
- Power of attorney in the case of an application completed by a proxy (optional)
proof of payment of stamp duty for a power of attorney (optional) - If you do not know if you are concerned about optional documents, see the service description
There are a number of option to submit the application:
- In person to the office (click here for address, opening hours etc)
- By post: National Labor Inspectorate Main Labor Inspectorate, Barska 28/30, 02-315 Warsaw
- Electronically, via online portal.
